List and photos of all menu items, including ingredients (will be asked to note commercially produced ingredients) and preparation.
General Liability Insurance ($1,000,000 Minimum)
Food Handlers Cards for all Individuals Involved
Commissary Kitchen / Health Inspection Report
Gas Permit Issued by Fire Department (if using propane onsite)
Fire Extinguisher with tag (Class K required if frying foods onsite)
